WOTC Instructions and Tips

File online or by mail using the links below.

• Use the IRS Forms 8850 and 9061 to gather employee and employer information.
  
• To file for the tax credit, a Federal Employer ID Number (EIN) number is required.  Click here to find out more about obtaining an EIN.
  
• Once you have all the required information, the employer can fill in the online combined IRS Form 8850 and 9061 ETA Form. 
  
• To complete the online form on behalf of the employer, complete the IRS form 2848 Power of Attorney and Declaration of Representative.  Once the Power of Attorney is approved, file applications and check their status here.
  

To electronically send support documentation such as your address, drivers license number, rehabilitation letters and/or military documentation you must scan the document, reference the person it pertains to and email it to:  des.wotc@illinois.gov​.
