1099-G Tax Form

Unemployment insurance benefits are taxable income. If you received unemployment insurance (UI) benefits last year, a 1099-G tax form is required to complete your state and federal tax returns. The form shows the UI benefits you received, and any taxes withheld.

Repayments on overpayments are not reflected in your 1099-G.

How do I Get a Copy of My 1099-G?

1099-G forms are available for download and mailed out by the first week in February. Click here to change your correspondence preference.

Mail: IDES mails paper copies of the 1099-G form claimants who opted NOT to receive their form electronically. The form will come via the U.S. Postal Service.

Tele-Serve: Claimants may also request a mailed copy of their 1099-G via Tele-Serve. Please call 312- 338-4337 and select main menu option 3, for claim status.

Electronic: Claimants who opted for electronic copies receive an email notification when the 1099-G is available in your IDES claimant portal.

In-Person: If you are unable to access the form online or through Teleserve and you have not received it through the mail, you can schedule an in-person appointment for pickup at one of the IDES local offices.  

5 Easy Steps to Access 1099-G Online

Even if you requested a mailed copy, you can access your 1099-G form any time once released on the IDES claimant portal.

  1. Log in to your IDES account.
    ILogin registration is required to access all IDES accounts, including your 1099-G online. If you need to reset your password within IDES's secure website visit the ILogin page and view instructions on troubleshooting.  
  2. After you have successfully logged into your IDES account, navigate to the dropdown menu titled Individual Home in orange. 
  3. Select Request 1099-G
  4. Be prepared to answer security questions. You will need to accurately input your Social Security Number (SSN), Date of Birth, Driver's License/State ID, First and Last Name
  5. If your responses are verified, you will be able to view your 1099-G form.

You can view your forms for the past seven years. Select the appropriate year and click "View 1099-G". This form can be printed at local libraries and American Job Centers.

Important information about your appointment, including documentation you will need to bring with you and arrival instructions, is below:

Identity Verification | Be sure to bring two forms of ID with you.  This can include a Driver’s License or State ID, payroll stubs, W2, or a utility bill showing your name and address.

Pickup | The 1099-G may only be picked by the individual it belongs to and cannot be given to anyone else.  This appointment is only to pick up a copy of your 1099-G only. If you need assistance with anything else, including unemployment insurance claims, you will need to schedule a separate appointment Please call IDES Claimant Services at (217) 558-0401 to schedule an additional appointment.

In Person Pickup

If you are unable to access the form online or through Teleserve and you have not received it through the mail, you can schedule an in-person appointment for pickup at one of the IDES local offices.  Appointments for pickup can be made Monday through Friday between the hours of 8:30am to 9:30am or 4pm to 5pm. 

These appointment slots are for 1099-G tax form pickup only.  Click the link below to schedule your appointment at the local office closest to you. If you need to cancel or reschedule your appointment, click on the ‘Reschedule’ button in the Booking Details of your confirmation email. Once on the page, be sure to select ‘Reschedule’ and not ‘New Bookings’ to adjust the time to pick up your 1099-G form.

Got 1099-G Form but Never Received UI?

If you didn't receive UI benefits and received a 1099-G form, this may indicate that a fraudulent claim was filed in your name. If this is the case, immediately contact IDES at (800) 244-5631.

Once a fraudulent claim is reported, investigated, and confirmed by IDES, the victim will not be held responsible for repaying any benefits fraudsters may have received in their name, nor will they be held responsible for tax implications resulting from a fraudulent claim.
